Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services.  Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials submittals. The following observations were observed on site this day.

Our representative was on site and observed proof-rolling of the subbase stone for the repair deficient asphalt areas on Mountain Ash Lane. The proof-roll was also observed by the Paulding County inspector and the grading contractor using a loaded tandem dump truck loaded with 19 tons. All areas appeared stable under proof-roll traffic with no severe movement which is now suitable for asphalt paving. Repair areas listed below:

1.Between house #148 & 132

2.Between house #170 &156

3.Between house #186 #170

4.Between house #223 &207 (isolated strip 18″x 38″) with movement, recommended clip and add additional asphalt.

5.Between house #171 & 189

6.Between house #145 &155

7.Between house #123 & 141
