Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site to observe the contractor (NG Grading) and for the observation/testing of the placement of fill/backfill.

The pipe crew continued installation of the water main through the site. They also continued the installation of SSMH 5.2 to 5.3. This area was backfilled with a mixture of blasted rock and PWR. Visual inspections were performed to observe the backfilling process. Density testing is unable to be performed due to the presence of large rock. The contractors also began excavation of AB-11 to AB-13.

The wall crew was on site and spread No. 57 stone and GAB into the remediation area in the footing of retaining wall 1. Due to the over-excavation of the remediation (by the grading crew) when spreading the GAB across the No. 57 stone, the material ran out before covering the whole area. The grading crew became frustrated and tried to blame the wall crew for spreading the GAB too thick. This was not accurate. The grading foreman requested ASEC to change our recommendation to accommodate for another change order to Pulte for the additional GAB. ASEC refused and contacted Mr. Wood to inform him of the situation. The grading crew also did not excavate enough area along the wall for the wall contractors to lay out their geogrid to the spec of the wall. The grading crew excavated approximately 18 feet in width, when the plans of the wall show 26 feet in width in this area. The grading crew is now having to remove this soil before the rest of the wall can be installed properly. The wall crew is installing baskets in the areas where they can properly lay out the geogrid. Density testing was performed and test results were provided to the contractor.
