Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site to observe the contractor (Lewis Contracting) for the observation/testing of the placement of fill/backfill. A proof-roll was performed on the roadways and parking lot areas of the site. The proof-roll was performed using a loaded tandem axled truck. One section of the heavy-duty entrance was unable to be rolled at this time due to a manhole blocking the roadway. Once GAB is placed up to this manhole, the trucks will be able to roll over top of it without risking damage to the structure. The remainder will be proof-rolled either later this week or beginning next week. There were two areas that showed pumping during the proof-roll, and these spots were marked with paint and recommended to excavate approximately 1 foot deep and replace with base course stone. The contractor is continuing to pour curbs around the site. ASEC will return to visually inspect the remediation for the roadways before GAB is placed.
