Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site as requested by the contractor (Lewis Contracting) for the observation/testing of the placement of fill/backfill. The contractor excavated the remediation areas in the roadway and replaced with large surge stone. The contractor is starting to spread GAB across the previously proof-rolled areas that had passed. A visual inspection was performed on the remediation areas and ASEC was requested to evaluate the (light duty entrance, and the middle of the two heavy duty entrances). Once the contractor has placed GAB over the areas that have passed proof-rolling, they will inform ASEC when they are ready to proof-roll the remaining areas before placing the rest of the GAB. The contractors are still actively pouring curbs.
