Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site to observe the contractor (Lewis Contracting) and for the observation/testing of the placement of fill/backfill. The contractors backfilled the structure located along Main Street with GAB. The contractor used a jumping jack to compact the surface while ASEC was onsite, and then density testing was performed. Test results were provided to the contractor. The contractors are removing a section of the paved roadway by the grease traps located in the middle of the site. This area appears to have not been installed correctly and had not properly drained water from recent rainstorms. This appeared to cause the asphalt to hold water in some sections and crack apart. The contractor is adding more GAB before paving this section again. The grading contractors stated they believed the asphalt possibly got cold during the installation and that the pavers had rushed the install.
