Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services.  Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials submittals. The following observations were observed on site this day.

On the above date our representative observed the wall contractor (RWS) pouring the west side approach slab at the bridge abutment. The reinforcing steel was observed prior to placement and appeared to be in compliance with project specifications for size, spacing and quantity. The contractor poured approximately 29 cubic yards of a 5000-psi mix which only required a 4000-psi mix. The writer observed the placement and molded one set of six (4″ x 8″) test cylinders breaking one early break at 4 days.

The writer observed proof-rolling of subgrade soils at the Unit 4, Phase 2 main roadway to the cul-de-sac. The subgrade was proof-rolled using a loaded tandem dump truck loaded with 18 tons. The subgrade appeared stable under traffic except isolated areas noted, 1.12’x40x4″, 2.7’x30’x4″, 3.32’x10’x4″ and 4.25’x7’x6″. The contractor began placing base stone and clipping recommending areas and replacing with additional base stone.

The electrical contractor (Mastec) continued excavation and installing conduits for wiring.

Concrete data for placement: Slump 3.75-inch, product code 50CA6, description 5000 AEA, supplier Ernst, concrete truck no.489, ticket no.5335656, time batched 8:28 am, time sampled at 8:50 am. Location was west side approach slab at the bridge, sampled at 27 of 29 cubic yards. Temperature of air was 62 degrees, and concrete mix was 70 degrees.

We appreciate the opportunity to be of service to you on this project. If you have any questions regarding this report, please feel free to contact us. We will be more than happy to discuss it with you.
