Summary *

Our AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Vidal Dias visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to observe, verify and document the ongoing project activities. (Reference drawing E720661 S1 - R5, E720659 S1-R5.)

During the visit, the following observations were made:
<ol>
 	<li>One thousand nine hundred twenty-six (1926) cubic yards of fill materials were placed in section G8 (drawing E720659) of the basement. </li>
 	<li>The materials were placed in accordance with ITP-001. The area was probed with a 3/8" diameter probe rod, and two (2) soil density tests were performed in section 1, area B7 (E720661 S1 - R5), and section 2, area G8 (E720659 S1-R5). </li>
 	<li>A proof-roll was performed using a full tandem axle water truck in section 1 area B7 (E720661 S1 - R5); no significant deflection or rutting was detected. The proof-rolled area is highlighted in purple on the drawing. </li>
