Summary *

Our AS ENGINEERING AND CONSULTING, LLC (ASEC) representative, Vidal Dias, visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to observe, verify and document the ongoing project activities. (Reference drawing E720661 S1 - R5, E720659 S1-R5.)
During the visit, the following observations were made:

<ol style="font-weight: 400">
 	<li>One thousand two hundred ninety-six (1296) cubic yards of fill materials were placed for lift two (2) in section 1 area B7 (drawing E720661) of the basement. The materials were placed in accordance with ITP-001, and the fill area is highlighted in orange on the attached drawing.</li>
 	<li>The area was probed using a 3/8" diameter probe rod, and two moisture and density tests were performed.</li>
 	<li>A proof-roll was performed using a full tandem axle water truck in section 2 area G7 (E720659 S1 - R5); no significant deflection or rutting was detected. The proof-rolled area is highlighted in purple on the drawing.</li>
 	<li>No. 24 stone was placed around the HDPE pipe in section two (2) area G7. The stone was placed in accordance with ITP- 003.</li>
