Summary *

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Vidal Dias visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (reference drawing E720661 S1 - R5, E720659 S1-R5.)

During the visit, the following observations were made:
<ol>
 	<li>Two thousand one hundred sixteen cubic yards of fill materials were placed in sections 1 &amp; 2 </li>
 	<li>The fill material was placed in general accordance with ITP-001. The area was probed, and two density tests were performed in section 1, area B7 (E720661 S1 - R5). </li>
 	<li>Photo locations are labeled on the attached drawing. The placement area is shaded in purple on the attached drawing. </li>
</ol>
