Summary *

Our AS ENGINEERING AND CONSULTING, LLC (ASEC) representative, Vidal Dias, visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to observe, verify and document the ongoing project activities. (Reference drawing E720661 S1 - R5, E720659 S1-R5.)

During the visit, the following observations were made:
<ol>
 	<li>One hundred and twenty-six (126) cubic yards of fill materials were received for the purpose of lift four (4) in section 2 area G7 (drawing E720659) of the basement. The materials were placed in compliance with ITP-001.</li>
 	<li>A moisture test was performed, and the soils exceeded the required moisture limits. </li>
 	<li>A proof-roll was performed in the basement area H7 in section-1 at the ramp area.</li>
