Summary *

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Vidal Dias visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (reference drawing E720661 S1 - R5, E720659 S1-R5.)

During the visit, the following observations were made:
<ol>
 	<li>One thousand six hundred twenty (1620) cubic yards of fill materials were received and placed for lift eight (8) section one (1) area B7 (drawing E720661). The materials were placed in accordance with ITP-001, and the fill area is highlighted in blue on the attached drawing. </li>
 	<li>During the evaluation, two density tests and one proctor check were performed. The results are attached, and test locations are highlighted in red on the attached drawing.</li>
