AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Ram Mogulla visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (reference drawing E720661 S1 - R5, E720659 S1-R5.) During the visit, the following observations were made: <ol> <li>One thousand and eighty (1080) cubic yards of fill material were received and placed for lift 14 in section 1 basement area G7 (drawing E720661) of the basement. The materials were placed and compacted in compliance with ITP-001.</li> <li>Three density test and one check plug were performed in section 1 and section 2 basement areas B7 and G7 and the test locations are visible in the attached drawing.</li> </ol> We appreciate the opportunity to be of service to you on this project.
