Summary *

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Avinash Bandam visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (reference drawing E720661 S1 – R8, E720659 S1-R8.)

During the visit, the following observations were made:
<ol>
 	<li>One thousand (1134) cubic yards of fill materials were placed for lift (13) in section 2 part 2 area G7 of the second part (drawing E720659) of the basement. The materials were placed in accordance with ITP-001, and the fill area is highlighted in brown as depicted in the attached drawing.</li>
 	<li>The area was probed, and three moisture density tests were performed in the basement, section 1 basement area B7 and in section 2 basement area G7. The attached drawing depicts the approximate test locations.</li>
