Observations/Finding
Summary
*
As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials submittals. The following observations were observed on site this day. <p data-start="276" data-end="656"><strong data-start="276" data-end="302">Summary of Activities:</strong><br data-start="302" data-end="305" />A proof-roll was performed previously on the designated section of the Main Parkway Extension (see attached map). Two areas failed the proof-roll and required remediation. The first area measured approximately <strong data-start="504" data-end="528">60 ft x 21 ft x 2 in</strong> in depth; soil was removed as directed. Remediation of the second failed area was not completed during this workday.</p> <p data-start="658" data-end="927">Two off-road dump trucks delivered <strong data-start="693" data-end="724">Graded Aggregate Base (GAB)</strong> from the Dallas quarry throughout the day. Approximately <strong data-start="782" data-end="793">50 Linear feet</strong> of GAB was placed along the future road. <strong data-start="841" data-end="877">No compaction or density testing</strong> was performed on this material during this shift.</p> <p data-start="929" data-end="1214">In a separate work area along the Main Parkway Extension, a trench located near the centerline of the future roadway was being completed. <strong data-start="1075" data-end="1104">Manhole #4 and Manhole #5</strong> had been installed prior to arrival (unobserved), and <strong data-start="1159" data-end="1175">12-inch pipe</strong> was placed between the two structures. Backfilling operations were performed using a <strong data-start="1262" data-end="1287">Kobelco 350 excavator</strong> to place fill material. Compaction was achieved using a trench packer and a ride-on vibratory sheepsfoot roller (see photos). <strong data-start="1414" data-end="1437">Three 12-inch lifts</strong> were placed and compacted. Each lift was probed following compaction. Additional compaction was requested on each lift.</p> <p data-start="1587" data-end="1727">Work on this trench remained incomplete at the end of the day. The final lift left the trench approximately <strong data-start="1695" data-end="1726">1 foot below finished grade</strong>.</p> <p data-start="1587" data-end="1727">Drilling was completed by Advanced explosives (see attached drilling log).</p> <p data-start="1587" data-end="1727">No further observations were made on this day.</p>
