Observations/Finding
Summary
*
As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials submittals. The following observations were observed on site this day. On the above date our representative was onsite to observe reinforcing steel placement at the bridge barrier walls prior to concrete placement. The contractor placed approximately 36 cubic yards of a 4500 psi mix using a pump truck. The reinforcing steel was found to be in compliance with site documents for size, spacing and quantity after clearance discrepancies were corrected, specifically, wall chairs missing at some areas and the south side wall not having sufficient clearance from the form work. The contractor used a 2 x 4 to shift reinforcing steel allowing sufficient clearance while pouring. The writer also assisted in monitoring and sampling concrete. One set of (6) six 4 x 8 cylinders were cast for placement. The writer observed subbase stone at Naturewalk parkway phase 1 after a severe rain event. Severely eroded reels were observed at both sides of the curb lines which will require repairs. Also, several manhole structures were filled with washed silt, and some previous poured curbs were undermined requiring repairs. The writer recommended filling deep eroded reels with full depth GAB stone. The writer observed the grading contractor placing fill for Unit 4 Phase 3 being placed over saturated surface soils within the top two feet of subgrade between Lots #133 and 134. It was recommended that fill placement should not continue with the severe rutting conditions that now exist. We appreciate the opportunity to be of service to you on this project. If you have any questions regarding this report, please feel free to contact us. We will be more than happy to discuss it with you.
