Observations/Finding

The Airfield Spall Repair project site was visited by ATL QA representatives to monitor the construction activities. NLVR at Taxiway A13: Upon arrival contractor continued grading base course with a Catepillar D3 bulldozer. GAB appeared to be very uneven toward the eastside of the construction area. Also towards middle of the site several dips in the grade were observed and was noticed that proper grade had not been achieved. Notified HJDP Rep. Mr. Bill Stitley about the grade elevations. Contractor began installing formwork on the east end north edge. Contractor QC, HJDP Rep. Mr. Bill Stitley and ATL QA Rep. Mr. Gerald Walker questioned the formwork installed when the area is not on grade and not been tested. Contractor responded that the area had been tested by QC the previous night. ATL QA notified the HJDP personnel that test is not acceptable until the loose base course material is compacted and also recommended to check the grade. Contractor proceeded to install formwork and gaps were noticed under the formwork ranging from 1-inch to 5-inches. Contractor then realized the GAB was not on grade. Contractor removed the formwork and cancelled the pour. Towards the end of the shift contractor hauled in more GAB and motor grading operator to complete grading of base course.
