Observations/Finding

The undersigned AS Engineering and Consulting, LLC (ASEC) representative was on site as requested by Archer Western/Lewis Contracting to perform a visual inspection of the structure designated as CB-27.

Visual observation techniques were employed to evaluate work in-place for general compliance with the Project Plans, Specifications, Codes and Standards and Material Submittals.

We understand the above mentioned element is approximately 1-inch lower than its planned elevation. The cause for this discrepancy is unknown at this time. One of the theories is that this may be a result of equipment movement in the area after the structure had been installed. The contractor requested a visual inspection of the structure in place.

During our visit the inside of the structure was visually observed and several pictures were taken. Please refer to photographic documentation below, for additional details.  Individual images are available for your review if needed.

During our visual inspection, no cracks or other obvious signs of structural damage were noted. We understand a more comprehensive video record of the installed structures and connecting pipes is forth coming. This should provide additional confirmation of our findings. Based on our observations during this visit, no corrective action is needed.
