Observations/Finding

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Vidal Dias visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (reference drawing E720661 S1 – R5, E720659 S1-R5.)

During the visit, the following observations were made:

1. One thousand forty- four (1044) cubic yards of fill material was received and placed in sections one & two of the basement. The fill placement area is shown in orange on the attached drawing. The materials were placed in general accordance with ITP-001. 
2. The contractor placed 825B on the northeastern side of the gravel road near the first riprap berm. The aggregate was placed and compacted with a walk-behind gas plate compactor. The area is highlighted in purple on the attached drawing. 
3. Two moisture density tests were performed, and the results were acceptable in accordance with ITP-001.
