Observations/Finding

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Vidal Dias visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (reference drawing E720661 S1 – R5, E720659 S1-R5.)

During the visit, the following observations were made:

1. One thousand five hundred sixty-six (1566) cubic yards of fill materials were received and placed for lift six (6) in section 2 area G7 (drawing E720659) of the basement. The materials were placed in accordance with ITP-001, and the fill area is highlighted in blue on the attached drawing. 
2. Four moisture tests were performed during the placement. The moisture test locations are labeled in red on the attached drawing.
