Observations/Finding

As requested, our AS Engineering and Consulting (ASEC) representative conducted a site visit for DOA Fiber Vault #1 on Aug 7th. The primary objective of this visit was to perform quality control inspection and testing services using visual observation techniques to ensure compliance with project drawings/specifications, applicable codes, and materials submittals. The following observations were made during the site visit:

  1. Standing Water in Vault #1 Area: Upon arrival at DOA Fiber Vault #1, it was immediately evident that standing water was present throughout the vault #1 area. This standing water was especially noticeable in areas that had been previously excavated prior to a rain event. A photograph (see attached) was taken to document the extent of the water accumulation.
  2. Recommendation for Soil Remediation: Given the observed standing water in the excavated areas, a recommendation was made to the project team on-site. It was advised that the affected soil in the vault #1 area should be removed and replaced to address the water accumulation issue. This recommendation was made to ensure the integrity of the soil and to prevent any potential structural or environmental concerns in the future.
  3. Lack of Remediation at the Time of Observation: Regrettably, at the time of the observation, no immediate remediation actions were observed on-site. The affected soil had not been removed or replaced as per the recommendation.
  4. Quality Assurance (QA) Presence: It is noteworthy that the Quality Assurance (QA) team was present on-site during the observation.

In conclusion, the site visit to DOA Fiber Vault #1 revealed standing water in the excavated areas, prompting a recommendation for soil removal and replacement. However, as of the time of this observation, no remediation actions had been taken.
