Site Inspection Report
Prepared by AS Engineering and Consulting (ASEC)

Purpose:
This report summarizes the quality control inspection and testing services conducted on-site to verify compliance with project drawings, specifications, applicable codes, and material submittals. Visual observation techniques were utilized during this process.

Observations:

1. Proof-Roll Observations:
   • Location:
     • Entrance and stabilization of Retaining Wall #1 from Station 0+00 to Station 0+15.
     • Project entrance. (See attached pictures for detailed locations.)
   • Methodology:
     • A fully loaded tandem-axle dump truck was used to conduct proof-rolling with multiple passes across the designated areas.
2. Findings:
   • Yielding and deflection were observed in all tested areas. Specific affected locations and sizes are as follows:
     • 17′ x 12′
     • 15′ x 15′
     • 10′ x 8′
     • 41′ x 8′
     • 39′ x 8′
3. Recommendations:
   • The contractor was informed of the observations.
   • It was recommended to undercut approximately 6 inches of soft soils in the affected areas and replace them with compacted graded aggregate base (GAB).
