• Flowable Fill Installation: Observed the placement of 32 cubic yards of flowable fill around two 1,500-gallon grease interceptors.

• Spall Preparation: The contractor removed loose rock, debris, and standing water from the base of three spalls to prepare for repairs.

• Reinforcement Bars: Rebar within the spalls was found to have epoxy coating, with embedment lengths noted as per specifications.

• QC Sampling: Sampling for the spalls was not performed during this shift, as Manhattan representative Timothy Rutledge indicated it was unnecessary.

• Spall Dimensions: Three spall areas were documented with the following dimensions:

  • 22” x 9” x 5.5”

  • 22” x 12.5” x 5.5”

  • 30” x 12.5” x 5”
