Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the
purpose of providing quality control inspection and testing services. Visual observation techniques were
employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site as requested by the contractor for the
observation/testing of the placement of concrete. Concrete testing was conducted from the middle of the sample out of the first concrete truck on site. The concrete appeared to have a high moisture content. Slump test was recorded at 8.5in slump (as shown in picture) due to this high moisture content. The contractor was informed of test results. 5 cylinders were made out of this sample and will be transported into the lab once they have cured.
