Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site to observe the contractor (NG Grading) and for the observation/testing of the placement of fill/backfill.

Late afternoon on the previous workday, the wall contractors said they were ready for testing on the first basket of retaining wall 1. It was explained to them to continue further down the wall and start on the other end of the wall so testing could be performed the next morning. The contractor agreed. Upon arrival to the site, the contractor had instead began building up the next basket over top the 1st basket on the same end of the wall. When asked why they continued installation over top of the first run, they had no explanation. It was explained to them that testing was required on each basket of the wall (as was done on the previous 4 walls) but the contractor continued without proper testing. The wall crew spent the day building up one side of the wall (approximately 5 baskets high) in a section of approximately 50ft to 100ft long. The grading crew and ASEC has asked multiple times why they are not doing full runs across the entire wall. There has been no reasonable explanation. The wall contractors appear to not have the proper equipment to place/compact the lifts. The grading crew is using their dozer and sheep-foot roller to spread the soil and compact.

Lifts for the wall were spread approximately 8 to 12 inches thick and compacted with a vibrating sheep-foot roller. Density testing was performed and test results were provided to the contractor.

The pipe crew began installation of sewer line 3.1 to 3.2. They are installing the clean outs and have not yet begun backfilling the line.

The grading crew is spreading the additional 1 foot of soil across the commercial lots. Soil lifts were installed in approximately 8-to-12-inch lifts and compacted using an 815 sheep-foot roller.

The drillers performed a blast on the previously drilled holes. Both pages of the drill logs are included in the pictures below. When ASEC requested the blasting log, these were the only two sheets provided. Page 1 consists of 41 holes and Page 2 consists of 88 holes drilled in an 8′ x 8′ grid pattern with a 3.5-inch bit.
