Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services.  Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials submittals. The following observations were observed on site this day.

On the above date our representative was on site to observe the concrete pour at the north side bridge embankment mud mats for the foundation. The contractor placed a total of 116 cubic yards of 3000 psi concrete for a total of eight strip footings numbered 1, 2, 4, 5, 6, 7 and 8. See attached sketch. Our representative assisted in molding three sets of (5) test cylinders for compression strength testing. Concrete index testing (air content, unit weight and slump tests along with air and mix temperature checks) was performed. The concrete sampling and results will be in a separate report. The footings not poured were number 3, 9, 10 and 11. A concrete pump truck was used for placement.
