Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site to observe the contractor (Lewis Contracting) and for the observation/testing of the placement of fill/backfill. Upon arrival to the site, the contractors shut down traffic on main street to prepare for a concrete pour over the two water lines that run across main street into the site. It appears the contractor placed approximately 3 feet of GAB over the pipe and compacted the GAB with a trench roller and a jumping jack. Density testing was performed and test results were provided to the contractor. The concrete pour in this area is approximately 3 feet to 4 feet wide and 1.5 feet to 2 feet deep across both lanes on the road.
