Observations/Finding

As requested, the site was visited by our AS Engineering and Consulting (ASEC) representative for the purpose of providing quality control inspection and testing services. Visual observation techniques were employed to verify compliance with project drawing/specifications, applicable codes, and materials
submittals. The following observations were observed on site this day.

Our ASEC representative was on site to observe the contractor (Lewis Contracting) and for the observation/testing of the placement of fill/backfill. The contractors are installing a structure located along Main Street. At this time, there has been a hold up due to fitting issues and the contractors are waiting on approval to change the design of the install. The fitting issues are due to the structure running along the property line of the private residence nearby and of the water main that runs along Main Street. The contractors are going to temporarily backfill the structure with No. 57 stone until a decision is made on how to proceed forward.
